Billing and Insurance
Responsibility for payment
For our patients' convenience,
TheSleepLab participates with and, in many cases, direct bills to
a large number of insurance programs. However, the primary and ultimate
responsibility for payment rests with the patient.
Payment at the time of service
At the time of service, evidence
of current insurance coverage, in the form of your insurance card,
is required as is payment for any co-pay or co-insurance you may
be responsible for based on your plan. TheSleepLab accepts cash,
check or Visa and MasterCard as payment methods. If you do not have
insurance coverage for the procedure being performed, payment in
full for the testing will be required at the time of testing unless
arranged with our office in advance of your visit. To find out the
charges for the testing you are scheduled for, please call our office
during normal business hours.
Referrals and preauthorization
Most insurance programs require a referral from
your doctor and/or preauthorization by the insurance carrier in
order for insurance coverage to be provided. Your referring physician
should provide you with the written referral order. TheSleepLab
staff will assist you in obtaining any necessary preauthorization.
Please remember it is always the patient's responsibility
to assure that proper referrals and any required preauthorizations
are obtained. If you are not sure of what you need, please contact
your insurance company or employer's benefits manager.
Insurance programs with which we participate
We participate in some or all of the traditional
indemnity, PPO and POS programs of the following insurance companies
and the HMO's of the insurance companies marked with an *:
- Aetna*
- Anthem (Trigon)
- Carefirst
- BlueCross/BlueShield network
- Medicare
- NCPPO
- Tricare
- Optimum Choice
- MAMSI
- PHCS
If your insurance company is not listed,
please contact the billing department
to see if we have recently added it to the insurance programs in
which we participate.
As participation in programs changes from
time to time, it is recommended that you confirm with our office
at the time you schedule your appointment about whether we are currently
participating with your specific plan.
